South East Toronto Stroke Network

Within the South East Toronto Stroke Network (SETSN), the Regional Stroke Centre is located at St. Michael’s Hospital (SMH), Unity Health Toronto.  The SETSN and SMH are responsible for providing leadership in the development, implementation and coordination of stroke care throughout the region and across the continuum of care.  The Regional Stroke Centre provides emergency medical management of stroke/TIA, specialized inpatient acute stroke unit care, and urgent and non-urgent secondary stroke prevention services and follow-up.

Meet the Stroke Nurses

The SETSN Nurse Practitioners (NPs) are advanced practice nurses who operate from the Regional Stroke Center located at St. Michael’s Hospital (SMH). They work collaboratively with physician colleagues and the interdisciplinary health professional team to deliver comprehensive care throughout the patient’s medical journey. They are authorized to diagnose, order and interpret diagnostic tests, as well as prescribe medications.  They also provide leadership and clinical support within the stroke program and to the broader health care community in order to advance best practices in stroke care.

Meet the Stroke Assessment and Treatment Team

The Stroke Assessment and Treatment Team is an interprofessional team providing occupational therapy, physiotherapy, speech-language pathology, nursing, and nutritional counselling with a focus on improving clients’ functional abilities so they can live as independently as possible.  They provide leadership and education to support development and implementation of best practice stroke care related to the assessment, rehabilitation, and recovery needs of persons with stroke in the hospital and community.  Their role facilitates access to nursing and allied health stroke expertise and consultation services for patient assessment and treatment planning as required.

Meet the SETSN Executive Committee

The SETSN Executive Committee provides leadership and supports the implementation and continuous improvement of an integrated system of care within the stroke region. It contributes to the Toronto Stroke Networks Steering Committee’s mandate for the oversight and coordination of a broader Toronto stroke system.  The SETSN Executive Committee provides regional advice and perspective to support the shared strategic plan and workplan of the TSNs.
