When the person with stroke is ready to move on to rehab, provide the appropriate transition letter and confirm:

• Person with stroke’s and alternate contact information
• Where they will be living during rehab (for those going to outpatient rehab)
• Transportation to outpatient rehab program.  Provide options and a list of services available if required

During your conversation with the person with stroke and/or family/caregiver:

• Emphasize the importance of attending rehab
• Explain what to expect, helping reduce anxiety
• Prepare patients to actively participate in their recovery
